Completing the Online Application for the Distance Track.

DICAS Online Application: Register with DICAS and complete the online application. DICAS is the Dietetic Internship Centralized Application System (DICAS) which is used by most internships. The program may be accessed at https://portal.dicas.org

Reference Letters: When completing the online application form, applicants must include the name and contact information (specifically an e-mail address) for each reference. This will trigger an e-mail message requesting completion of a reference form. The form will be completed on-line. Students submitting more than one application will need to use the same individuals as references for each application.

Personal Statement: Applicants who apply to internships using DICAS will be asked to complete a personal statement. Content may include personal or professional goals, interests, experience, strengths, motivation or other topics deemed relevant to the selection process.

UHDI puts emphasis on work experience and would like to know about ALL work experiences that you consider relevant to your application for internship. Please feel free to include that information either on the form or in your personal statement. The program gives preference to candidates with DPD GPA of 3.4 and higher, science GPA of 2.5 and higher, and overall GPA of 3.0 and higher.

Verification Statement: All applicants must include an original signed verification statement from an accredited Didactic Program indicating that all required coursework has been completed. Students currently enrolled in classes must submit a "Declaration of Intent" listing courses in progress. Successful applicants must supply signed verification statements and final transcripts to UHDI prior to the start of the internship.

DICAS provides for your DPD director to login and submit the verification statement of declaration of intent statement. Please verify with your DPD director that they have done so.

Resume: A current summary of your academic, employment and professional activities. Please include your email address.

Rotation Schedule: Completed rotation schedule for all your prospective sites. Suggested start dates of first week of March (fall applicants) or first week of August (spring applicants). Rotations may not start prior to internship orientation (2nd week of January or 3rd week of July). Rotations may be scheduled in any order. Start date can be flexible as needed to accommodate site schedules.

Preceptor forms: signed agreement forms from each preceptor. Preceptor Qualifications form from each preceptor.

Application fee: A $50 non-refundable application fee is required. Check or money orders only. Checks should be made out to University of Houston, with the subject line as: DISTANCE INTERNSHIP.
