Program Completion Requirements
It is the policy of the UHDI to issue verification statements to all interns who have successfully completed a minimum of 1000 documented hours of supervised practice, completed the exit exam with a score of 80% or better, and have submitted all required DI app portfolio entries and uploaded documents within 60 days of successful completion of all supervised practice rotation hours.
Additionally, all borrowed resources must be returned.
Verifications will be emailed to the intern approximately 3 weeks after program completion when all completion requirements have been verified. Original signed copies of the verification statements remain on file at UH indefinitely.
